Important notes:

• You must click the "Submit by Email" button on the form to complete registration or save the registration form on your computer and email it to griffijm@forsyth.cc as an attachment.
• Print the form for your records.
• You must have Adobe Reader 8.1 or newer to view and fill out the form. It is free and can be installed in a couple of minutes. Click the link above to go to Adobe's website.

• Please read all materials carefully and fill out the application completely.

• Please make note of the total amount due.

• Applications should be submitted on-line by November 21, 2008.

• All reservations will be confirmed by return email.

• A confirmation with detailed direction will be sent by letter by January 2, 2009.

• All spaces are assigned on a “first-come-first-serve” basis.

• The Association reserves the right to arrange spaces to accommodate the facilities features, life safety codes and other needs to enhance the exhibits.

• The number of truck spaces per vendor is limited in order to accommodate multiple vendors.

• At this time we only accept Visa for online payments.
